MOHR is More: Integrating Patient Reported Health Experiences in Primary Care Settings

Researcher presents findings from a randomized controlled trial to test the feasibility and impact of fielding My Own Health Report (MOHR), a behavioral and mental health risk assessment and goal setting tool for use in primary care. Outcomes included strategies practices developed to field MOHR; the proportion of patients completing MOHR; the frequency of unhealthy behaviors and mental health issues in primary care; and the impact of MOHR on counseling, goal setting, and patient reported health behavior and mental health.
